The Hospice Hub provides comprehensive, compassionate, and integrated services to support individuals at end-of-life including their families and caregivers. Our programs and services include home support, day hospice, and grief and bereavement support. Our talented and dedicated volunteers are at the heart of delivering our programs by providing companionship to our clients, respite to caregivers, bereavement support to groups and individuals. We assist with system navigation and advocacy for clients and caregivers. We serve Perth, Smiths Falls, Portland, Westport and surrounding areas. Our catchment area is that served by the Perth and Smiths Falls District Hospital.
