Private Pay clients or direct referral from VCH Home Hospice Program (Primary contract holder for all in-home palliative care nursing in Vancouver)

Your hospice palliative care program/service provides support and/or care for the following:

Cancer
Cardiovascular Disease
Infectious Diseases (ie HIV/AIDS, Hepatitis-C)
Neurological Disorders
Renal Disease
Respiratory Disease
Care for all conditions

How would you answer the following question if asked by a patient, client, resident and/or family? “What can you do for me?”:

We assist families with navigating the challenging medical system and educate them on services available through the public sector.

Provide in-home bedside nursing services to support clients and families at end-of-life in symptom management, medication management and comfort care. We can provide respite to families for day or night shifts, or 24/7 RN care if needed. Our RNs can complete 1 hour visits for additional needs such as injections and wound care.

If RNs are not yet required in the home, but assistance with personal care, meal preparation, homemaking, medical escort and companionship are, we have a vast team of qualified Care Aides to assist for as little as 3 hour shifts to 24/7 care.
