Essential Conversations: A Guide to Advance Care Planning in Long-Term Care Settings
Residents of long-term care homes deserve to receive care that reflects their values and wishes, and to participate in decisions about their lives and care. In an ideal world, people moving into a long-term care setting would already have engaged in Advance Care Planning (ACP). But that is not always the case.
