Now Accepting Nominations for the CHPCA 2012 Award of Excellence

Do you know someone who has made an outstanding national contribution as a CHPCA member and who exemplifies professional and/or personal commitment and achievement in the field of hospice palliative care in Canada? Why not nominate them for the CHPCA Award of Excellence!
 
Award Eligibility

The nominee is an exemplary CHPCA member whose excellence is devoted to hospice palliative care in Canada in either a paid or voluntary capacity. To be a recipient of this award, the nominee must have made an impact at the national level and contributed to the mission of the CHPCA by enhancing the quality of life for individuals and their loved ones who are facing a life threatening illness and bereavement.

The award will be presented to only one individual in a given year and it is possible that it may not be granted every year.

2011 Nominees Please Note

In recognition of the time and effort requried to prepare a submission, the Awards Committee is offering 2011 first time nominees the opportunity to qualify for a 2012 award. Following the nominees permission, nominators are asked to review eligibility criteria and submit a request for reconsideration as an addendum. Please include all recent accomplishments and any other pertientent updated information.

A completely new submission is required for individuals who have been nominated two years in a row.
