The Yukon Home Care Program is a territory-wide service that works closely with other government departments, First Nations governments, medical facilities and community partners.
The program provides:
- acute;
- chronic;
- palliative;
- rehabilitation services; and
- respite care.
Services are based on an individual’s assessed needs.
