The Care Coordinator receives referrals from Family Doctors, meets with the dying person and his or her family, identifies the needs for support, and arranges for the volunteers to provide the required support.

Hospice services are available to every dying person and his or her family, regardless of race, age, sexual orientation, gender, financial status, social situation, or religious belief.

The Hospice Care Coordinator and volunteers are dedicated to providing excellent care for the dying person and his or her family in the environment best suited to the person’s unique situation. That may be the person’s home, the hospital, or a residential care facility.

We strive to maintain the dying person’s dignity, individuality, and quality of life at all times, by providing physical, psychological, emotional, and spiritual support wherever necessary.

We are able to maintain support throughout the time of dying as well as in the family’s bereavement.
We maintain confidentiality for the person and for the family.

We work to educate health professionals and community members about hospice care and its part in the support of the dying.
