has a terminal diagnosis is aware of their diagnosis and prognosis accepts that hospice care focuses on comfort and quality of life and not curing illness understands that CPR (cardio-pulmonary resuscitation) is not offered as a treatment has a documented do-not-resuscitate (DNR) order. has a valid Ontario Health Card has a life expectancy of weeks to a few months has symptoms that can be managed by Hospice staff is eligible for services through a Community Care Access Center (CCAC).
