CHPC Nurses Group Lifetime Achievement Award

We seek to honour an individual who:

  • Is or has been a registered nurse or nurse practitioner
  • Is or has been a certified hospice palliative care nurse in Canada (CHPCN(C))
  • Is a current member of the CHPCNG and has been a member for a minimum of 5 years.
  • Has demonstrated excellent hospice palliative care nursing and
  • Has made outstanding lifetime contributions to hospice palliative care
  • Has made an outstanding contribution at either local, regional, national or international level
  • Has made significant contributions to the CHPCNG.

For example the nominee is or has done any or all of the following:

  • utilizes the Canadian Hospice Palliative Care Nursing Standards in clinical practice (link to the standards)
  • contributes to hospice palliative care education
  • participates in, supports and/or utilizes current hospice palliative care research
  • serves as a leader and an advocate for hospice palliative care

Nomination Process

  1. Seek the nominee’s permission to nominate him or her (Self-nominations are ineligible). The nominee must give permission for the CHPCNG to use his or her photo and the reference letters for publicity purposes. The nominee must sign the nomination form.
  2. Fill out the nomination form from the CHPCNG webpage (click here).
  3. Procure and attach to the nomination form two letters of support. Letters can be from a member of the CHPCNG, a professional or a member of the public. Each letter should not exceed 500 words.  Please provide specific examples of how the nominee has made significant contributions to HPC Nursing and the CHPCNG, see above examples.  Letters should address these key points.  Please click here to view the key points.
  4. Obtain and attach to the nomination form one copy of the nominee’s current CV.
  5. Obtain and attach to the nomination form an electronic photo of the nominee which may be used for publicity purposes.
  6. Submit completed nomination form with supporting documents by March 15th.

Note:

  • The nominator does not need to be a member of the CHPCNG.
  • Only applications that have all the supporting documentation will be accepted for review.

Award Recipients will:

  • Be notified by a letter and/or a phone call by April 15th
  • Receive Lifetime Achievement Award Certificate and plaque
  • Receive free lifetime membership to the CHPCNG
  • Be honoured at the CHPCNG AGM and Annual Report, the CHPCA Conference, through social media and the CHPCNG Message Board
  • Have a notification letter sent to the award recipient’s employer, with his or her permission

Award Nominees will:

  • Be notified by a letter and/or a phone call by April 15th
  • Be mentioned at the CHPCNG AGM and Annual Report, through social media and the CHPCNG Message Board
  • Have a notification letter sent to the nominee’s employer, with his or her permission

CHPC NG Leadership Award

We seek to honour a hospice palliative care nurse who has demonstrated extraordinary leadership. The nurse may have demonstrated leadership through advocacy for high quality hospice palliative care, the generation and application of knowledge and research, establishment of collegial partnerships, and/or contributions to the professional development of others through education, consultation, leadership, and mentorship.

We seek to honour an individual who:

  • Is/was a registered nurse or nurse practitioner
  • Has demonstrated extraordinary leadership in hospice palliative care nursing.
  • Is a current member of the CHPCNG.

For example the nominee is doing or has done any or all of the following:

  • advocates for and promotes high quality hospice palliative care.
  • advances HPC nursing through the generation and application of knowledge and research.
  • establishes interprofessional relationships and collegial partnerships
  • contributes to the development of students, peers, colleagues, and others through consultation, education, leadership, and/or mentorship.
  • communicates and advances the distinct contribution of nursing within the interprofessional team.
