2021 Hike for Hospice Registration Process

Interested in hosting a Hike for Hospice Palliative Care?  Follow the steps below to sign up as a Hike Site.

 

Registration for the 2021 Hike for Hospice Palliative Care will be opening soon.  Please check back regularly for more details.

Step 1:

Download and read a copy of the 2020 Hike Site Participation Agreement.

Step 2:

Register to Host a Hike Site.

Click HERE to go to the Hike Registration Form.

Step 3:

Fill out the Host Site Participation Agreement and Waiver Form and submit it to the CHPCA.

Step 4:

Send the CHPCA a copy of your proof of insurance.
